Why we collect information about you?
Your therapist and the team of professionals supporting you keep the records about the treatment and care you receive from CCATS. These records help to ensure that you receive the best possible care. They may be written down (manual records) or held on the computer.
These records may include:
- Basic details about you e.g. Name, Address, Date of Birth, and Next of Kin.
- Any contact we have had with you such as clinic visits.
- Notes and reports about your health.
- Details and records about your treatment.
- Results of psychometric tests etc.
- Relevant information from people who support you and know you well such as health professionals and relatives.
Some of this information will be held centrally to be used for statistical purposes. In these instances we take strict measures to ensure that individual clients can not be identified.
How your records are used
Your records are used to guide and administer the care you receive to ensure that:
- Your therapist, support worker or other professional involved in your care has accurate and up to date information to assess the care you have received.
- There is a good information base for professionals to assess the type and quality or care you have received.
- Your concerns can be properly investigated if a complaint is raised.
How you can access your records.
You have the right of access to the information we hold about you on our records.
Your request must be in writing to Data Controller, CCATS, Milton Youth and Community Centre, Milton Street, Fleetwood, Lancashire. FY7 6QW.
You will need to give adequate information (e.g. full name and address, date of birth) so that your identity can be verified and your records located.
If anything is incorrect please let us know so the record can be corrected.
A 40 day response period is allowed.
How your records are used to help others
Your information may also be used to help us in the following ways.
To help us manage CCATS by being used to:
- Audit CCATS accounts and services.
- Investigate complaints and legal claims.
- Make sure our service can meet client’s needs in the future
- Prepare statistics on CCATS performance.
- Review the care we provide to ensure it is of the highest standard.
- To help reach healthcare professionals.
How do we keep your records confidential?
Everyone working at CCATS has a legal duty to keep information about you confidential.
You may be receiving care from other people as well as CCATS (e.g. Social Care). We need to share some information about you with them so we can all work together for your benefit. We will only ever pass information about you to them if:
- They have a genuine need for it under their organisational terms of reference.
- We have your permission
We will not disclose your information to a third party without your permission unless there are exceptional circumstances such as when the health and safety of others is at risk, or if the law requires us to pass on the information.
Anyone who receives information from us has a legal duty to keep it confidential.
Who are our partner organisations?
We may share information with the following main partner organisations:
- NHS
- General practitioners
- Social services
- Educational Services
- Voluntary and Private Sector Providers.
Notification
The data protection Act 1998 requires organisations to lodge a notification with the Information Commissioner to describe the purposes for which they are processing personal information.
